免責同意書入学時に所定のDeclaration（免責事項確認書）に署名することにより、学生、両親または法的保護者は、以下のことに同意するものとする。

の所有者、経営者、役員、従業員、並びに予約は取次ぎを実施した旅行会社は日本とオーストラリアの法律により
法的責任を免れる事象に関しては一切責任を負わない ものとする。 学生、両親または法的保護者はTom Fielding Golf
Schoolまたはそのプログラム運営の為の関連団体組織が所有、賃貸、運営、管理する場所において、
また、学生の為に手配された宿泊場所や授業の一環の 各種行事において起きた、または学生によって起こされた、学生の死亡、怪我、身体障害、損失、損傷、
所有物の損害に関してはいかなる請求の申し立てもしないものとする。さらに、学生もしくは学生が18歳未満の場合は、両親及び法的保護者は、 Tom
Fielding Golf School
やその代理人、もしくは認知した機関により組まれたプログラム（スポーツ、文化、社会的、レクリエ―ション、その他）において、またお客様が
出席、参加する種々の活動の場において、学校で負った直接的または間接的な費用を支払うことに同意する.

TOM FIELDING GOLF SCHOOL GOLF CAMP WAIVER AND RELEASE OF LIABILITY READ BEFORE SIGNING.
In
consideration of being allowed to participate in any way in the Tom
Fielding Golf School's 2025 Japan Summer Time Golf Camp program,
related events and activities, the undersigned acknowledges,
appreciates, and agrees that:
1) The risk
of injury from the activities involved in this program is significant,
including the potential for permanent paralysis and death, and while
particular rules, equipment, and personal discipline may reduce this
risk, the risk of serious injury does exist; and,
2) I KNOWINGLY
AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF
ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full
responsibility for my participation; and,
3) I willingly agree
to comply with the stated and customary terms and conditions for
participation. If, however, I observe any unusual significant hazard
during my presence or participation, I will remove myself from
participation and bring such to the attention of the nearest official
immediately; and,
4) I, for myself and on behalf of my heirs,
assigns, personal representatives and next of kin, HEREBY RELEASE AND
HOLD HARMLESS the Tom Fielding Golf School, Japan. their officers,
officials, agents, and/or employees, other participants, sponsoring
agencies, sponsors, advertisers, and if applicable, owners and lessors
of premises used to conduct the event (“RELEASES”), WITH RESPECT TO ANY
AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or
property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASES OR
OTHERWISE, to the fullest extent permitted by law.

HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT,
FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL
RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY
INDUCEMENT.
